Rethinking Implant Site Development: From Vertical Augmentation to Partial Extraction Therapy

Presentation Synopsis

Long-term implant success depends not only on osseointegration, but on the preservation of crestal bone and peri-implant soft tissue architecture. While vertical augmentation techniques have expanded the reconstructive possibilities in implant dentistry, long-term stability and predictability – particularly in the esthetic zone – remain challenging.
Historically, implant therapy has focused heavily on restorative and surgical hardware modifications, including implant design, connection geometry, surface characteristics, and platform switching. Most recently, attention has switched toward biologically driven approaches, aimed at preserving the patient’s native tissue architecture rather than reconstructing what has already been lost.

This presentation will review the limitation of vertical augmentation procedures, discuss the biologic rationale behind partial extraction, therapy, and explore contemporary strategies for transitioning patients toward implant retains restorations, while maximizing aesthetic and regenerative outcomes.
