For decades, success in fixed prosthodontics has largely been defined by survival rates. Yet restorations that remain in function are not necessarily free of biological, technical, esthetic, or patient-centered complications. As biomaterials, digital workflows, and restorative concepts continue to evolve, clinicians are increasingly challenged to distinguish true long-term success from mere persistence over time.

This lecture will critically explore the contemporary evidence on tooth- and implant-supported fixed restorations, drawing from systematic reviews and long-term clinical outcome data. Emphasis will be placed on the factors that truly influence clinical longevity, including material selection, biologic integration, complication profiles, maintenance requirements, and patient-related outcomes. The presentation will also examine the gap between innovation, marketing claims, and evidence-based performance.

By moving beyond survival alone, this lecture aims to provide clinicians with practical, evidence-based strategies for achieving predictable and durable success in fixed prosthodontics.
